According to The Institute of Healthcare Improvement (IHI), medication dosage errors and drug interactions are often the result of inaccurate or incomplete information relayed at hospital admission, transfer or discharge, and sometimes during doctors' visits. As part of IHI's 100,000 Lives Campaign, Doylestown Hospital is developing a comprehensive, computerized medication tracking system. The Medication Tracker is an ideal way to personally present a list of your medications to your doctor or pharmacist.
The Medication Tracker is available in all Doylestown
