For people who need hands-on care at home, Hunterdon Regional Community Health (HRCH) is available to help you. HRCH, a certified, nonprofit alliance of home health providers offers professional home-based medical and supportive services to assist you with aging parents, family members with temporary disabilities or to care for someone with a chronic, progressive illness.

“HRCH offers access to high-quality home care services to meet individual needs,” says HRCH’s executive director, Joan Grady, RN, MSN.
HRCH is comprised of five partner agencies: Hunterdon Medical Center Home Health Services, Hunterdon Hospice, Visiting Health and Supportive Services, Hunterdon Infusion Services and Lifeline® Personal Response Service.
Each agency works in conjunction with a patient’s physician to develop an individualized care plan to provide health and supportive services using a personalized team approach.
Team members include skilled registered nurses, physical and occupational therapists, speech pathologists, dietitians, medical social workers and certified home health aides. Arrangements can also be made for infusion therapy services, home delivery of medications, medical supplies, equipment and short-term respite care. The hospice team also provides spiritual and bereavement counseling and trained hospice volunteers.
If a patient’s safety is a concern, HRCH’s NeighborCare service can provide companions for safety and supervision. A personal response service is also available through the Hunterdon Lifeline program.
“Many of our clients say that they could not have cared for their loved ones at home without the supportive services offered by the HRCH staff,” adds Ms. Grady
