When a loved one is facing a terminal illness, a strong network of support can help alleviate pain and fear. In this moment of need, Pottstown Memorial Medical Center Hospice Program can be your network of support.

The skilled team – including, medical director, hospice manager, home health aides, hospice social worker, volunteers, bereavement coordinator and a chaplain – provide physical, emotional and spiritual support. Like PMMC’s Home Care patients, hospice patients remain comfortable in their own home while receiving care.
Affiliation with PMMC ensures that the program meets strict quality standards. PMMC’s hospice is the only program in the area accredited by the Joint Commission.
The hospice team works with the patient, their personal physician and hospice medical director to develop a plan of care that meets the patient’s individual needs for pain management and symptom control. Members of the team make regular visits to assess the patient’s status and provide additional care. The staff is on-call 24 hours, seven days a week. For more information please call 610-327-7216.
